West Coast IPA Case Study
In 2022, an independent practice association within the West Coast IPA region of California utilized Caret Health to boost its value-based performance, lower total cost of care, and capture additional revenue. The target demographic was 5000 patient lives, a large portion of which are high risk patients with multiple chronic diseases.
Program
The program had 4 aims:

Increase the IPA’s aggregate STAR ratings under value-based arrangements

Screen and identify patients with social determinants gaps, and make corrective clinical actions to address these gaps

Leverage the IPA’s digital health infrastructure to utilize preventative care, leading to a reduction in expensive hospitalizations and ER visits

Risk adjustment
Reconcile data from multiple sources of truth to optimize risk factor management and improve coding
Caret’s platform and implementation team helped the IPA set up a centralized command center of medical assistants, nurses, and providers. This centralized team utilized the Caret platform to aggregate and reconcile data, identify gaps in value-based performance, and execute clinical pathways to close performance gaps.
The IPA was able to visualize progress of key processes such as earnings forecasts, screening, clinical pathways, task management, patient engagement, and staff utilization. This brought much needed transparency and reconciliation to its digital health portfolio.
Outcomes
After 1 year, the IPA achieved 4+ star ratings on key quality measures and an additional 10% cost savings below 2021. This generated a 17x improvement in value-based revenue: over half a million dollars per thousand patients that was previously left on the table.
